Divergences in detail
The reviewer's APPROVE was invalidated by later same-session commits to the reviewed PR
- What the prose says: the reviewer reviews
<ISSUE-KEY>"and nothing else. When you have finished it, stop and report." The skill has no step that edits, commits, or pushes code — its whole contract is review + record a verdict ("Never merges anything";allowed-tools: Bash, Read, Grep, Glob— note: noWrite/Edit). - What happened: the review completed cleanly and APPROVED PR #111 at
14:01:25(f57887c1). Then, starting at the user's14:13interjection, the session pivoted: the user ran the JST-122 feature's own pipeline, found runtime bugs, and directed a series of fixes/enhancements. The agent editedfeature_report.ps1andcollect_feature.ps1, updated docs, added report sections and mermaid pie charts, and committed + pushed five times tofeature/JST-122-conversation-feature-report(14:27push, then commitsd83a3f7,7256d45,efd2f38,69f7e63) — all part of PR #111. - Consequence: PR #111 now contains substantial code the reviewer never reviewed; the posted APPROVED verdict is stale — it approved a diff of 8 files/+771 that was subsequently rewritten by five more commits.
- Likely why: the user commandeered the session for a different job after
the skill run had already finished. Per the debugger's own caveat, "an agent
obeying the user over the skill is not a divergence." This is user override,
not a
jira-task-reviewercompliance defect — the skill correctly ran to completion and stopped first. - Suggested fix: no skill change. The reviewer did its job and reported; what followed is the user's prerogative. Worth flagging only as an operational caution for the humans: a reviewer session that then edits and pushes to the PR it just approved leaves an approval that no longer matches the code, and a fresh reviewer re-run against the final diff would be the clean way to re-bless PR #111. (This run happens also to be the one whose transcript is the data source for the very feature it was reviewing — a self-referential setup, not a skill problem.)

All three reviewer-scoped errors were self-inflicted syntax/plumbing in the agent's own verification harness (it chose to run the code live as part of the Correctness dimension), each recovered within one retry with no residue and no effect on the verdict. No skill script or prescribed command failed.

Verdict
Within its own scope the run is a clean, high-compliance single-step review:
correct Windows dispatch, faithful step-1→6 walk, idempotency check, full-diff
read, an APPROVE posted to both GitHub and Jira via --body-file, status left
for the automation, and a correct S-APPROVED chat report — the agent even ran
the code end-to-end to back its correctness call. The one thing to act on is
operational, not a code change: after the review finished, the same session
was redirected by the user into fixing and pushing five commits onto the very PR
it had just approved, so the standing APPROVED verdict on PR #111 is now stale
and the feature should get a fresh reviewer pass against its final diff. That is
user override of a completed run, so the finding lands on the humans/workflow,
not on jira-task-reviewer's prose — the skill text needs no fix here.
